D. Renal & Hepatic Conditions

  • ESRD

  • Hepatic failure

E. Wounds & Skin Conditions

  • Pressure ulcers
  • Non-pressure ulcers
  • Gangrene
  • Skin infections in hospice

F. Debility & Failure to Thrive

Hospice-specific caution:

  • R54 (senile debility)
  • R62.7 (failure to thrive)

These cannot be coded unless supported by documentation.

Hospice Documentation Requirements

Candidates must understand how to interpret:

  • CTI (Certification of Terminal Illness)
  • Face-to-Face (F2F) documentation
  • Narrative describing terminal prognosis
  • Decline indicators:
  • Weight loss
  • Functional loss
  • Increased symptoms
  • Poor response to treatment

Hospice Sequencing Rules

1. Principal Hospice Diagnosis

  • The condition most contributing to terminal decline
  • Not always the earliest disease
  • Not always the hospital discharge diagnosis

2. Secondary Diagnoses

  • All related diagnoses must be coded
  • Impact symptom burden and care planning

3. Unrelated Diagnoses

  • Must be documented as unrelated by the hospice physician.

  • Code only conditions supported by documentation
  • Avoid financial bias
